COVID-19 Vaccine Appointment
We are taking appointments for COVID 19 vaccines now. Vaccination appointments are scheduled online only.
Eligibility:
The state of Florida requires that those receiving their first vaccine dose provide proof of residency meeting certain criteria. The following Florida residents are eligible for the COVID-19 vaccine, by appointment only:
- Healthcare personnel with direct patient contact
- Residents and staff of long-term healthcare facilities, and
- Adults age 40 and older beginning March 29
- Adults age 18 and older beginning April 5
- Children Age 12 and older beginning May 10th
Furthermore, if the following contraindications apply to you, we discourage you from scheduling a vaccine visit as we will not be administering a COVID-19 vaccination to you.
- Current illness
- COVID-19 diagnosis or illness in the last 90 days
- COVID-19 antibody treatment in the last 90 days
- Any vaccination in the last 14 days, including the flu shot
